Trouble with express-scripts.com Login

Just updated to 3.9.12 Safari 1Password extension and still no love for https://www.express-scripts.com/ .

I'm sorry that you're having this problem. Please try saving a new Login manually on the actual login form like this:
- Visit the site's login page: https://host1.medcohealth.com/consumer/site/ulpLogin
- Enter your credentials, but DO NOT submit the form.
- Click the 1Password button in your browser's toolbar, and select the "+" button in the upper right hand corner.
- Change the title and make any notes (if desired).
- Click the Save button in the upper right hand corner.
Using the above technique I was able to save and fill a sample login at:
https://host1.medcohealth.com/consumer/site/ulpLogin
Manually saving a new Login can be useful for logins that are either problematic to begin with or were once working but have since stopped. Saving a new Login item allows 1Password to refresh everything it "knows" about the page. Login pages often change as websites are updated and this can be a necessary but very useful tip. :)
If you are still having trouble, please try disabling autosubmit for that Login and let me know if that resolves the issue. Since I don't have an account there, I was not able to verify autosubmit for the form.

Still no success... I've tried a few times now. Restarted safari and everything. Each time after manually adding a new login, the 1Password menu remains empty.

The problem is that the frame is located on a completely different domain, so 1Password's anti-phishing protection won't try to fill it on the URL shown in your screenshot. That's why I asked you to save a Login on the actual login page located within that frame:
https://host1.medcohealth.com/consumer/site/ulpLogin
You'll also need to fill it there. You can do this easily by selecting the Login from the "Open Login in…" section at the bottom of the extension popup. To locate it more quickly, you can simply begin typing the first few letters of the Login title to narrow down the list in the extension popup.
