Saving credentials doesn't always create an entry

I don't think I've seen this mentioned elsewhere - I'm finding that new website credentials are not always stored in my keychain after hitting save in the chrome extension pop-up. The first few times this randomly happened, I assumed I must have hit 'not now' by accident but I've been double-checking my actions and can confirm this is not user error. The chrome pop-up goes away and there is no error message or any other indication that the save didn't work.

I notice the problem when I try to log back in to the recently saved site and find no saved entry in the list or I try to edit the saved login via the Mac 1Password app and there's no trace of an entry. When this happens, if I rinse and repeat, I find it's always stored the second time around with no change to any app versions or my environment.

I'm wondering if this could be related to keychain locking timeouts?

Please clear your cache in 1Password (Help > Troubleshooting).
