BMO Business - three login fields

Bank of Montreal (BMO) Business site has three login fields.
(https://www22.bmo.com/ctpauth/CTPEAILogin/CustUserPasswordAuthServlet?TAM_OP=login&ERROR_CODE=0x00000000&URL=/pkmslogout&HOSTNAME=www22.bmo.com&AUTHNLEVEL=&PROTOCOL=https)

Customer ID, User ID and Password.

1Password on Mac saves only the userid and password.

How can I force it to also save the Customer ID field?

1Password Version: 6.6.4
Extension Version: 4.6.4.90
OS Version: OS Sierra
Sync Type: Dropbox

    Manually saving a new Login item for that site should work. Instructions are here:

    Even though I don't have an account, I was able to create a test Login that filled all three fields on the Sign In form:
