BUG: When adding an item to a folder, there is a button to create a new item instead of Cancel

A quick summary of the bug:

Try to add an item to a folder. Instead of a Cancel button on the right side of the navigation bar, there is a + button. You can actually add a new item to your vault via this button, but the app crashes when you save it. After you open it again, the item is in your vault and even in the folder.

Detailed Step-by-step instruction on how to reproduce it:

1. Open 1Password, unlock vault
2. Tap Organize
3. Tap a folder (create one if necessary)
4. Tap Edit
5. Tap +. A panel shows displaying All Items with a + button in the navigation bar (http://cl.ly/image/3t1y471o0c1v). There should be a Cancel button instead.
6. To cancel, tap Categories, then Cancel
