Chrome Extension not working correctly

Extension version is 4.2.5.90, Chrome version is 38, 1Pass version 4.1.0.526 on Win8.1. I've uninstalled/reinstalled the extension, rebooted, and restarted the helper process to no avail.

Issues are:
- Left clicking the extension icon does nothing
- Ctrl-\ is not matching ANY urls

The IE plugin is working fine, and correctly matching the same urls and it's occurring for logins that only have one url saved, and when I manually choose that login to open in a new tab it goes to the same url that it won't match the login for.

Ideas?

Have you tried any of the tips in the "Troubleshooting" > The browser extensions article in the 1Password 4 for Windows knowledgebase?

Temporarily disabling web browser code signature verification has been solving some extension problems recently.

Help > Advanced > Verify web browser code signature --> OFF
