Hello Cloaky,
Definitely different. Never seen that before.
Were you able to reproduce this behavior, or was it a one-off experience? Out of curiosity, what version of OS X are you running?
You may be the winner of the "Odd Screen Shot of the Day" award.
Love to hear more about this one.

Hey Cloaky,
Thanks for sharing this.
Could you please provide some additional information?- What version of Mac OS X are you running?
- What version of 1Password are you running?
- If you were not already using 1Password 3.6.1 at the time of the incident, are you able to reproduce the issue after updating to 3.6.1?
- What steps can I take to reproduce the issue on my own machine?
Looking forward to your answers to those questions so we can figure out where the problem actually is.
Cheers,
UPDATE: I was able to reproduce this in Mac OS X 10.7 with 1Password 3.6.1 by simply bringing up the "Add New Item" sheet (File > New Item > New Account) and then pressing ^⌘L to lock 1Password. It is just a UI glitch (not a security issue) since attempting to add an item after the lock results in an error since the data file really is locked.
I have passed this along to the developers for resolution in a future release.0 - What version of Mac OS X are you running?
