1Password Mac app launches every time I turn on my Mac

Starting about a year ago, the 1Password app started launching every time I turned on my Mac — The full window, asking for my Master password. I have been using the app for several years, and the app never launched unless I opened it myself.
If I right-click the icon in the dock, "Open at Login" is not checked.
If I go to my System Prefs, 1Password is not included in the list of Login Items.
I uninstalled the Chrome extension, but the app still launches.
It started doing the same thing on my wife's mac at about the same time it started happening on mine.
It's becoming a nuisance. Any idea how to make it stop, other than deleting the app entirely and switching to LastPass?

    @npyritz: Thanks for reaching out. I’m sorry for the confusion! 1Password is incapable of starting itself on login, so something else is causing this. Are you telling your Mac to "reopen windows when logging back in" when you restart or shutdown? Also, do you have other apps that launch on startup — for example a browser— which could be opening 1Password as well? Let me know what you find!
