iphone passwords not synced with mac

My one password file on mac is not the same as on the iphone. I rarely use it on the phone, but today the mac died and when I tried to get passwords off the phone, the app on the phone would only accept an OLD master password. And the individual entries are similarly out of date.

Also, when trying to log in on the phone it asked for a secret key, which I don't know anything about. I found an alternate login path (cannot remember what) which only wanted a password, where I tried the current one, and then in desperation, the old one.
